Workaround Fix
1st fix: Deactivate application and submit a new one with the same option selected.
2nd fix: if first fix fails. Deactivate the application. Ask candidate to reapply by ticking another box.
Root Cause
Root cause is currently not known. Possibly flawed logic in the Umbraco form. Updating forms in Umbraco has adverse effects. Tracked under Request SR-192.
